An above ground permanently installed pool. Where does the 5 Ft. measurement for pool equipment I.E. pump motor start. Does it start at the top (water edge) or the bottom of a pool?

Any point of the pool. Measure from the outlet to the closest point of the pool (top, bottom, middle of the side, metal ladder, whatever).

680 wording is "5 ft. from the inside wall".
To me it only makes sense that this is a strait horizontal line from the pool. If you were to measure from the waters edge and you had a 5ft. wall on the pool the receptacle could be on the pool wall edge. That just doesn't seem right to me.
