bare ground conductor to panel

it seems my interpretation is a little vague here. when creating a Ufer ground with a bare solid conductor, would a bond bushing and /or connector be required typically 1/2 ko into the panel or could the conductor enter through a existing 3/8 ko directly to the buss ,less the bushing? where in art 250 for spec please

The small holes are there for the entry of the GEC, no connector or bushing required.
