If a second circuit is run to a bathroom for lighting only in residential does it need to be 20A? There is already a 20A circuit run to the required receptacle.

It does not. 210.52(D) 20 amp requirement is for receptacle circuits.

Even though it is not required to run a 20 amp circuit we usually do because alot of the exhaust fan/light/heater combos out there require a 20amp circuit.

If the existing 20 amp only goes to that bath , the light can go on the same circuit . If the 20 amp goes out to another bath recep the lights can not go on the circuit . Also you dont have to run a circuit to the lights, they can go on any general purpose light circuit , it does not have to be 20 amp

The fan/ light /heaters usually require a separate circuit and cannot be added to the lighting circuit whether it is 20 amp or not. At least that has been my experience in the past. Here are the specs for a Nutone Unit
