I have steam generator with nema 50R receptacle that is single phase 240V phase to phase, 50A in bathroom. I cannot understand the language first paragraph would it require gfci nec 2017 section 210.8(b)? My understanding is that it 120V single phase 50A would require ground not the one I have but not sure language says 150V to ground

208 is 120v to ground so if its "other than dwelling" it needs GFCI.

The language in the 2017 code is totally wrong as receptacles are not rated as the section suggests they are. The language was corrected in the 2020 code to read "All 125-volt through 250-volt receptacles supplied by single-phase branch circuits rated 150 volts or less to ground, ..."
That being said the intent of the section is as stated in the previous comments. If the branch circuit that supplies the receptacle has a voltage of 150 volts or less to ground, the GFCI protection is required.
