Battery egress light maintenance record

Does anyone have a form that they use for testing battery egress lights- the monthly and annual tests?
I tried looking in 70B but did not see anything except for battery strings.

We have a work order system to track when this maintence should be done and then use a label much like you get when you change the oil in your car. Simple paper label that states date and who checked it.
