I would say the box itself has no rating. The cover that is used on it will determine the rating for the assembled device. I would think very few in-use covers would be type 4.

All I see on the package is "Listed 22FO". If a blank cover or lever type switch cover is used would that put it in the NEMA 4 catagory?

I am bringing this up after working in a class II, division II, group G location. There was a bell box with lever switch cover in this area running a fan for summertime air flow. The fan's motor is rated for this area. At the time I thought maybe because the box is gasketed it is ok to have in this area. NEC 2011- 502.10 (B)(4) : All boxes and fittings shall be dust tight.
