Best way to add a 200 amp breaker to this panelboard

I've been called in to fix a big mess left by another "electrician" who was "cash only" and bailed out on a small restaurant in the middle of upgrading an overloaded 100 amp subpanel to 200 amp. I think he bailed when the time came to figure out how to actually get the 200 amps out of the existing main 400 amp 240v single phase panel among other terrible messes he got himself into.

The existing NQOD30L400CU main panel has room for a QO2200 (Graybar's recommendation) but I prefer my 200 amp breakers bolted down. I asked for a bolt in option and he said they are discontinued. I do alot of service work and have seen so many loose QO breakers that are barely holding on to the busbar over the years I am very leery of installing a 200 amp one. Am I overreacting? Really I am just looking for a different solution to this problem. I've attached some pictures of the main panel for clarity

Jim Dungar can verify for sure when he's on site but I would look at a NQSFL2 Sub feed lug kit
