Bonding Bushings

Under what conditions are bonding bushings required to be used? I thought it was based on the conduit size or whether or not concentric knockouts were used. I can't seem to locate it in the code. Any help out there?

Its just more reading ... But 250.94 answers the question of the Week !

Frankly the boss showed up on the job and asked about a bushing, and I said, Cause I can ... :)
