I am trying to see where it says in NEC, maybe a picture of how after a transformer, ( separately derived system), you bond the XO in the transformer, the first disconnect is 100' away, does the disconnect get bonded?

Your profile states EI, you should know the NEC does not have pictures, for that you will need the NEC handbook.
It would be challenging to explain in words what you are asking.
I would also suggest you get a copy of Mike Holts grounding or Understanding the NEC.
However a simple explanation is at the XO you install a SBJ and then connect to a GEC.
The disconnect is bonded to the EGC, which goes back to the XO.

If this indoors the 100' is a problem.

Simple answer: No, you would not bond ground to the neutral again, ever. To do so would place the neutral and EGC in parallel.

You can bond the neutral either at the transformer or at the panel but as Larry stated never both. We always bond the neutral at the transformer. As texie stated your 100 feet of secondary conductors is probably a violation.
