I don't know if there is a limit or not but I did see a former employee stack 7 11/2 4 gang ext in rings on a vertical mounted box to accomadate some ice cube relays for an automatic door operator

I have not seen anything in the code book that would address this issue. If you extend the conductors out from the pipe 6 inches you could only add 1 ring. If it is a box extention I would say you could only add one extention. If you added 2 extentions then you would be extending an extention ring. In either case you should add an equipment ground if there is a device installed on the ring.

Two places of reference for this situation could be, 300.14 Length of Free Conductors at Outlets, Junctions, and Switch Points. This could definitely restrict how many, and the listing of the particular extension collar you intend on installing.
I decided to look a little further and here is another possible section.
314.22 Exposed Surface Extensions. Surface extensions from a flush-mounted box shall be made by mounting and mechanically securing an extension ring over the flush box . (This is from the 2002 NEC, the italics are from me)
The UL White Book reference is QCIT, page 81 in the 2002 copy.
"Box Extensions - Box extensions are suitable for extending properly secured flush or surface mounted boxes. One or more extensions may be used."

I guess that answers the question quite well.
