Box sets back from face of wall - options?

I've done a search, and can't seem to find a post on this topic - but I'm sure there is one - just don't know how to refine the search.

Have run in to this numerous times, on old work. Box sits back in the sheetrock various depths. Those things that Gardner Bender makes (called Quick Fix Spacer), that stack up to support behind the receptacle yolk are real neat - but are they legal? I thought I remember reading something that indicated that the surface of the box could be no more than 1/4" back from the surface of the rock - is that the case? Also, if it's over that, are you actually forced to cut it out and affix the proper depth plaster ring, or some other solution. Remember seeing also a picture of a device that was basically the same opening as a "device box" and they came in various depths.

Is this gap only critical on a wall that has a fire rating - like between occupany spaces or say in a residence - between the garage and the living space?

I just use the Arlington BE1 sleeves.

Found these - a Carlon solution - but I can't find any info on Carlon's site, as to what the holes are for that are on opposite corners. Anyone used these before, so they can straighten me out on how to use these? The supply house I deal with has Carlon, and I also think Home Cheapo also. I have a job I'm pricing now that will need these for sure - just about every box sits back anywhere from 3/8" to 1/2".

Bob (iwire) brought up an interesting point regarding the legality of using these in a fire rated wall. I didn't see any reason why you couldn't but we ended up agreeing to disagree.

Goldstar - thankss for the link. I read through and still can't find any info so that I would be clear on how these box entenders are installed. I'm struggling with what the two holes are for that are "kitty corner".

Also, do these things actually "float" inside the box? Or do they come in various depths - like a "mud ring" does? Sorry, but I really want to get a clear picture on how to use these.

It appears that this box extender is available in only one size. I think you might be better off using the one Marc showed in his post. I think Carlon also makes devices called "box gadgets". They are available in various depths from 1/8" to 3/4" and you can stack them to make up the difference.

Brett, the Carlon extender is made from the same basic mold as the low-voltage old-work ring (with the flip-out ears on screws), so they had no reason to omit the holes. The only difference is that the device holes are enlarged so the 6-32's will slide through them.

Be aware that these extenders may not fit in everyone else's boxes, since Carlon obviously used their own boxes when engineering them. They're rigid and closely shaped to the interior of the standard Carlon 1-gang box.

The Arlington extender is available in one- and two-gang versions, and the two-gang can be butchered and combined for three- and four-gang boxes. I like the Arlingtons the best. The corners are slit, so they can taper for easier insertion (minds out of the gutters, guys!), and they flex around devices.
 
The Carlons are only good for PVC boxes. They won't fit into a metal raised adapter cover on a 1900 box.
 
They arlingtons are great. You just slip them around the device, and screw through them. The front edge will sit against the wall finish, shielding the space between the wall face and the box edge. They basically float inside the box, as you asked.
