That information has been in the UL Orange Books for quite awhile along with what constitutes the membrane for the particular T and F ratings. I don't see FM mentioned either.

Yeah, its been around for quite some time. I remember in another thread a while back someone asking for a pic of the code. Thought it would be nice to share with others since I found it in one of my books.
