What would cause a circuit breaker not to trip when there is a direct line to neutral fault?

SquareD QO 115, relatively new loadcenter and breakers. Didn't measure the run but would estimate under 50'

I could see FP, Zinsco, etc but not QO.

Poor neutral connection between there and the panel?
 
Did an upstream breaker open instead?

Was it line to neutral for sure? Or possibly line to ground?

I've had situations where there was not a neutral to ground bond and a short to ground would not open a breaker.

Put another breaker in the same place. Than try it again. If it trips you know it was a faulty breaker. If it does not. Than it may be some thing else.
If it has ever been real wet that will cause corrosion to build (glob) up on the internal spring.
