Breakers on busways accessibility

Are breakers mounted on busways required to be readily accessible the same as breakers in panels?
I have seen busways with breakers mounted high above the floor several times but am not sure if it is allowed by current codes.
Is this allowed? Code section?
 
I believe so, see 404.8(A) exception 1, 2005 code. Breakers and fused switches are permitted to be installed at the same level as busway. Suitable means shall be provided to operate the handle from the floor.

What do they mean by "Suitable means shall be provided to operate the handle from the floor"? Is that a portable ladder?
 
A hookstick would be a suitable means. Possibly a cable or a rope that could open the breaker or switch from the floor. IMO a ladder does not meet the intent of the exception.
 
How else would you mount a buss head on an overhead buss duct (factory, manufacturing facility) if it weren't mounted overhead on the buss duct?
