Buit in place junction box

I need assistance in locating a code reference dealing with a contractor building a junction box in place with a structural beam running through the interior and no metal on the side against the concrete ceiling with penetrations through the concrete to an enclosure above? Also doesn?t such an enclosure require being listed?

I agree that the box must be listed and cannot be built on site. Look at 110.2 & 110.3
 
Our shop has built large troughs in concrete under MCC's without issues in the past. They are planning on doing it again on a job coming up. Now you have me curious if this practice is technically a violation or not.

There is nothing in article 314 that requires junction boxes to be listed, there are some that need to be listed.
Paddle fan boxes, boxes covered by aggregate, boxes constructed by thinner materials and boxes for damp or wet locations.

The box described by the OP is not required to be listed, only marked by the manufacturer which could be as simple as marking it with a sharpie by the guy who built it.

110.2 and 110.3 is general, 314 is specific to boxes so it is the section that should be used.
