If a busduct is routed to a MV switchgear, does it have to be TOP fed into the switchgear? Or can it be bottom fed?

I am not aware of any standard that requires top fed BUT if it is bottm fed it could produce a safety hazard and should be clearly labeled bottom fed.

I have been involved with them both ways so there is probably not a code prohibiting bottom entry but it sure gives the electricians fits at the low voltage chamber access unless you come into the side and bus duct elbows get expensive , it also incurs more support requirements on those 90's.

It's not an NEC issue. It's up to the manufacturer. You could check some of their web sites, to see if they supply gear with a bottom feed arrangement for bus duct. I have not encountered one myself.
