If this MIGB is required for emt how would it get installed . A regular set screw or comp connector would be installed and this would be installed on the threads of the connector ?

yes, it goes on the threads of the connector. it must be installed before conductors are pulled.

Typcially you would remove the lug, spin it on and then reinstall the lug. When the KO is near the back or side wall of the enclosure the lug will not allow you to spin it on. You may be able to spin the connector into the lug.
