Bushings

Our company has a policy of using plastic bushings on all conduits, sealtights, cord connectors etc. I have had employees inquire whether or not this is a code requirement.
I know it is a code requirement for all types of rigid threaded conduits. It does mention need of a bushing "unless the design of the box, fitting, or enclosure is such as to afford equivalent protection".
Is there any specific mention of factory listed sealtight fittings, (non-insulated) cord connectors, EMT connectors requiring bushing by code??
 
Here is a rule from art. 300.4(G). There may be other codes that may also apply in different scenarios but I can't think of any.
NEC 2008 said:
Insulated Fittings. Where raceways contain 4 AWG or larger insulated circuit conductors and these conductors enter a cabinet, box, enclosure, or raceway, the conductors shall be protected by a substantial fitting providing a smoothly rounded insulating surface, unless the conductors are separated from the fitting or raceway by substantial insulating material that is securely fastened in place.
Exception: Where threaded hubs or bosses that are an integral part of a cabinet, box, enclosure, or raceway provide a smoothly rounded or flared entry for conductors.
Conduit bushings constructed wholly of insulating material shall not be used to secure a fitting or raceway. The insulating fitting or insulating material shall have a temperature rating not less than the insulation temperature rating of the installed conductors.

These also achieve their intended use much better when installed **before** the wire is pulled; as opposed to being neatly placed afterward like when landing the panel. ;)

We always put them on before a pull... then when the tugger pulls the head of a large pull into the enclosure and snaps the plastic ones or pulls the rubber out of the grounding bushing, we replace them. :D We gotta get a better mouse trap!
