BX cable and fire

How many of you have seen the "old" BX cable and a ground fault be the cause of a fire?

I have had two separate instances here in the city that I inspect for that the fire department has called me out to look at that the BX cable was the culprit.

I have not seen but have heard the old BX that didn't have the thin wire could have issues when the cable was used as a ground and there was a fault. It will glow red.
