Can a junction box be used as a receptacle box....

If the cover of the 4x4 is made to accept a receptacle or switch it is acceptable. Not only acceptable but used alot with the inclusion of receptacle cover or mudring. Internally if properly bonded and cables properly secured, should not be an issue. The only concern might be the plastic cover plate where edge protrudes beyond the box edge can be caught on and ripped off or broken, switching to a metal cover would be appropriate. And given aesthetics are not an issue internal of the cabinet metal cover should not be objectionable from an appearance perspective.
 
Looks like a flat mud-ring (or whatever it is officially called) was used and then receptacle with faceplate.
not very professional looking but it gets the job done.
An RS cover would be my preference.
 
Out of curiosity, does anyone know if the listing documents or product standards define a metal box as "device"," junction", etc? I know if you look at the label on the cardboard boxes containing metal boxes they often say things like "switch box" or "junction box" but I assume that is just marketing terms.
 
Hard to tell but the receptacle placement does not look symmetrical in the “plaster ring”? Hack work? TR ?

I think that's just an artifact of the camera being slightly off center. It does suggest to me that it's a raised mudring rather than a flat mudring.

Good find. Or good artwork!! That would be easy to make from a regular blank plate. Which would be functionally fine, but NEC kosher?
Those are standard listed flat adapter covers. Come in various sizes, 4", 4 11/16," round, etc.
