Ceiling fan switch control model number / manufacturer?

Hi, I suspect this switch is bad. I'd like to replace only the switch and not the control module above the fan as the fan is hard to reach. Does anyone know who makes this switch? Will I have to "pair" the new switch to the existing control module in the fan canopy?

Is there not a manufacturer name on the side of the switch? Like, "CASABLANCA".

I think you just need to suck it up and go up to the canopy and see what you have for a module or what brand of fan you have for that matter.
