What is the physical distance required for access to a 480v 3 phase breaker and a 208 single phase breaker. Also is the distance straight out or a radius.Is the code universal or is it different in California?

When you say "acess" do you mean 'working clearance"? If so the MINIMUM working clearance in front of any panel, switchgear, transformer, etc 600 volts or less is between 3 and 4 feet, depending on the voltage and condition. Also the minimum width in fron of such equipment is 30", See NEC 110-16 for more explanation.
