Class 2 Wiring for Lighting

Can you run class 2 cable behind walls in every US jurisdiction to power low voltage light fixtures? Can the cord grips recess into the wall as well? Is a junction box required behind each low voltage fixture or can the cable come directly from the fixture, but held by a plastic bushing, into the wall space? Article 411 and the related articles seem to cover this well, but do New York, Chicago, and Las Vegas allow this construction? Are there others that I might be missing?

I would say the NEC would allow this if it is really a class 2 lighting system. Just because it is low voltage does not mean it is class 2. It seems like many assume if it is low volt it is a class 2 circuit.
