Code minimum residential service ground requirements

I've been trying to answer this quest: What's the code minimum grounding requirements for a residential service (say 200A)?.

Is it two 5/8" cu 8ft ground rods spaced 6ft apart with #6 cu run to each?
Do you have to bond to steel?
Do you have to bond to water pipe?
Do you have to provide something for LV? If I have an exterior panel with a ground bus.

What are the code minimums?
