what is the the code requirement for protection of the fixture/lamp in a walk-in refer. or freezer I.E. jelly jar and or cage??????

That is most likely a product standard, not a code issue. If the box comes without a fixture and you are installing one, then some common sense needs to be applied based on the size of the box and location of the fixture in the box.
Subject to physical damage, etc...
