The receptacle has to be a 110.3(B) violation. I doubt very much that the installation instructions include this particular lash-up.

The one in the wall even looks like a double slotted duplex recep without the ground to boot.

Yes, they're both quads. I used to play with them when I was a kid.

When I was 12, I ran power to my parents' shed and wired it.

The house of a family friend was wired throughout with those quad receptacles in the living room, dining room and bedrooms. No EGCs.
I guess if you wanted to plug in a lot of floor and table lamps they might have let you avoid multiple taps and extension cords.
