Code Violation?

Does anyone know where in the 2014 NEC that this installation is a violation. This is a 120 /240 volt, 100-amp Zinsco service. Underground feed. Outdoor panel. This customer ran a 120-volt, 30 amp cord through the lower cover of the service. The cover plate covers the service entrance conductors.

Four off the top of me head:

-Mixing of service and load conductors (this is a bit of a grey area, since it's a panel).

-That connector is not listed for use in a damp/wet location.

-That connector is not listed for use with SO cord.

-SO cord used in lieu of another wiring method for permanently installed equipment.

The connector is not weatherproof. Not rated for the cable. No wiring method is allowed in any COVER. Is the cord Weatherproof?:jawdrop:

where does it say that in the code?

using the cord is quite possibly a violation. the other stuff associated with the use of the cord like the style of connector and the type of cord is possibly also a violation but really a consequence of using a wiring method that might well be a code violation.
 
If that is cover is the utility section then Not allowed by the utility no matter what the NEC says.
