Code Violations?

Someone sent me a picture of this load center for a 1400 sq. ft. condo and was asking ME to point out any code violations.
Here's the ones I could find.
1. Electrical Tap over blank space.
2. Possible overload of panel with split type breakers. He didn't take a picture of the actual amperage rating of the load center bus but is going
try to find out.
3. Possibly imcompatibility of using square D, Cutler Hammer breakers
on a Challenger load center.

Does anyone else see any other code violations?

The panel is suitable for twin breakers and I would guess that it is a 125 amp loadcenter. I have no idea what it is fed with so I am not sure of any other violations.

Sure looks like that buss got hot where the tape was covering!
Panel door says use only Sylvania or Challenger breakers.
Unless there is a main in front of the panel it violates 6 throws of the hand rule.

I didn't notice the breakers but it could be those Sq. D's are listed for theat panel. I know Seimens makes breakers that are listed for Sq. D. Now I am not sure how that works.... they fit and we are told they are listed.. but
