Combined neutral and EGC for service entrance conductors

For service entrance conductors coming from a utility transformer can the equipment ground conductor (EGC) and netural be combined as one conductor on a 3-phase service entrance?

I know with a seperately derived source that wasn't a service entrance you would need to have a seperate neutral and equipment ground conductor but I was wanting to know what was allowed for a service entrance case?

The neutral in the service entrance conductors is the effective ground fault current path. (See 250.92(B)(1))
