Compression Lugs on MV Cables

I'm sure this is a simple question, but I'm just sort of confused. I've spent a while searching old questions on this forum and just haven't found an answer.

I know you're supposed to use the 75C column of Table 310.16 for cable ampacity determinations (even if your cable is 90C cable) because of 110.14(C)(1) and the fact that the vast majority of terminations are 75C.

What about for MV cables? There aren't 75C columns for MV cables, and 110.14(C)(1) only references LV cables. When you put a stress cone on do you have to use a 90C compression lug? If so, where do you find them? And how come many contractors use the same lugs for MV cables as for LV cables?

A quick look at the Burndy catalog shows that many of their copper uninsulated compression connectors are UL listed for use at 90?C and up to 35kV
