Concrete slab penetrations

We have a 2nd floor that wil need some penetrations in mezzanine floor. Anyone have a favorite sleeve to install before concrete is poured? Or ideas for penetration in floor slab between floors.

The dimensions involving the apparent cap portion near the top are unclear.
Never mind, I see from the AD that it is intended for stacking of multiple units for deep pours.

You would just drill into metal decking?
