Do conductors temperature, insulation, and voltage rating all have to be the same to be installed in a raceway? Thanks

No. As long as your calcs for temp are using the lowest rated conductor or for the voltage, use the lowest rated conductor. Typically, the voltage is not a consideration as in this context all the conductors will be 600 volt rated.

Generally no, but note for parallel installations, the conductors must have the same insulation type, 310.10(H) (not necessariy ALL conductors, but within each phase)
