I understand how 4" box and conduit is run within a block wall but what about if the wall is cast in place concrete. Could the box and conduit be run within the pour or it WOULD HAVE to be surface mounted? If it could be run within the wall how would it be supported so it wouldn't shift during the pour

It can be within the pour or installed surface mounted after the forms are removed. Within the pour the raceway is tie wired to the rebar and the box usually gets mounted flush to the inside of the form with screws or nails.

Tilt-up walls are more and more common these days as the forms can be built on site, poured, and lifted and set by cranes. As Infinity stated, you will need to tie your conduits to rebar. It's important to duct tape the entire box (I also do every coupling even if they are compression type.) When it is planned and laid out nicely, it makes a great installation.
