conduit openings in poured walls

What is the best way to leave an opening for pvc conduits in poured concrete walls?

Sometimes I see PVC sleeves through the wall, sometimes I see styrofoam placed in the form.


Then again, I see a lot of core-boring too.

Styoroam wrapped with duct tape.

Foam block-out is probably fastest, easiest, most economical way, but may not meet spec for the wall.

In formed walls I use PVC sleeves, fast and easy.

In poured decks I use Styrofoam.
