Does this "LB" requirement only apply to the utility portion ... meter and above or also meter and below ?

I have not seen anything in the NEC that prohibits service entrance conductors installed in conduit bodies. I would venture to say this is a POCO requirement.

Agreed. I'd say it only applies to the line side of the meter, to minimize stealing power, and based on the drawings.
 
Common utility requirement on the line side of the meter. However the language this utility choose to use, prohibits conduit bodies between the meter and the service equipment
