I have a project that falls under the NEC 2002. The project inspector is stating that underground concrete pullboxes with metal covers need to have the covers grounded per 314.28(C). 314.28(C) refers me to 250.110 which states "....fixed equipment LIKELY to become energized...". My opinion is that the pullbox cover is not "likely" to become energized as the splices are insulated and sealed with resin kits of the like.
