Control wiring that failed to stop motors?

Does anyone know of a case where motor control wiring did not stop a motor when the wiring failed? I know this sounds weird, the only thing I can think of was when an interlock failed and the motor kept running.

Anything is possible. This would happen with old 240-volt control circuits where one of the 240 legs would go to ground and the 120v could not pull the coil of the starter in but would hold it in

There are case of long 3 wire type control circuits where the stop button does not cause the starter coil to drop out because of the control wire shunt capacitance. Square D has a document on this.
