I am confused on when to apply note 3 vs. note 4 in table 220.55? If I have one cooktop and 1 wall mounted oven do I use note 3 and add all together and use derating of each, or do I use note 4 and add together and use as 1 unit? Any thoughts on this? I am preparing for the NEC exam and need to know how to answer the questions for the test. I am taking this exam in 1.5 wks and need to clarify this for the practice.
 
I would use the latter, Note 4. It seems to apply more specifically to your situation. Note 3 appears to apply to several of the same type of appliance.
 
This from the commentary following 220.55 Note 3, in the '05 NECH

The branch-circuit load for one range is permitted to be computed by using either the nameplate rating of the appliance or Table 220.55. If a single branch circuit supplies a counter-mounted cooking unit and not more than two wall-mounted ovens, all of which are located in the same room, the nameplate ratings of these appliances can be added together and the total treated as the equivalent of one range, according to Note 4 of Table 220.55.
