With cord hung lights, how do you deal with old work when installing boxes? Obviously studs will not always be at the right location. Are Round Old Work boxes sufficient?

An old work fan bar box would work well.

That depends on the weight of the fixture. Most old work ceiling boxes are marked "not suitable for fixture support". There is one brand, which I forget, that says it is suitable for fixtures up to 6 pounds. As a general rule, old work ceiling boxes are not suitable for fixture support. The old work fan boxes or some box affixed directly to the joist is your option.
