Cored Hole Conduit necessary?

I dont see any issue running it right thru with no additional conduit. I might put some sort of bushing or extra protection around it at each end in case it gets pressed up against the corner of the hole by backfilling or settling.
Yeah, I was thinking about grouting in a big bell ends.

Yes, it is. I just wasn't sure if there was some provision in the NEC that says "where susceptible to damage" or "direct burial cables shall not pass through holes in concrete without sleeves" or something like that .

No such requirement for cored holes so sounds like you're good to go.
