My understanding is that a light switched at the entrance would only be required in a crawl space if the craw space is to be used for storage, or if it contains equipment needing service per 210.70(A)(3). The handbook has a commentary that gives examples of equipment requiring service as air handling equipment, cooling and heating equipment, water or sump pumps. I am not sure what you mean when you say "utilities", but it may not fall into this catagory.
Similar requirements would apply to the receptacle, but it is not required for a storage space (210.63).
