I direct wired a 30a 240v coffee maker for the cafe owner (she wanted it like that) and am wondering if you need a disco for commercial. There is an on/off switch behind it. It came with a 6' piece of cord but no cord cap.

You may be able to use this:

422.34 Unit Switch(es) as Disconnecting Means. A unit
switch(es) with a marked-off position that is a part of an
appliance and disconnects all ungrounded conductors shall
be permitted as the disconnecting means required by this
article where other means for disconnection are provided in
occupancies specified in 422.34(A) through (D).

(D) Other Occupancies. In other occupancies, the branchcircuit
switch or circuit breaker, where readily accessible
for servicing of the appliance, shall be permitted as the
other disconnecting means.

I think that would depend on the function of that switch. If it is just a little toggle / snap switch on the CONTROL circuit of the coffee maker, and is not actually switching BOTH LEGS of the 240V input, it would not qualify as a disconnecting device (i.e. "all ungrounded conductors").

Hence the word "may". :)

Breaker lock sounds good. 422.31(B).

(B) Appliances Rated over 300 Volt-Amperes. For permanently
connected appliances rated over 300 volt-amperes,
the branch-circuit switch or circuit breaker shall be permitted
to serve as the disconnecting means where the switch or circuit
breaker is within sight from the appliance or is lockable in
accordance with 110.25.
