Disconnect on the secondary side of the transformer?

Is a disconnect required on the secondary side of a transformer? The panelboard is located 60' away and has a main breaker. The primary side has a non-fusible disconnect on it and is fed from a 60 amp breaker.
 
I believe at 60 feet you need a disconnect where the conductors receive their supply. This is not my forte so lets see what others say.
 
In 99% of the situations, a disconnenct/OCP would be needed on the transformer secondary within 10 or 25 ft {See 240.21(C)} for details}
On two-wire primary/secondary, delta-delta, or outside secondary conductors, the rules could allow greater distances if the transformer has proper primary overcurrent protection per Art 450.3
