Disconnect Switches

I have a situation where the electrical engineer has designed to install a series of bus plug disconnect switches (vertical bus riser) above the NEC 404.8a required maximum of 6'-7". I want to have the contractor provide a suitable means of disconnect, as discribed in exception 1. In this case, does exception 2 over rule exception 1 , so that the "suitable means" does not have to be provided? Does exception 3 over rule everything as long as a hook stick is provided?

If you have the handbook, see the article 100 definition of Accessible for examples of disconnects.

Exception 2 can be applied without regard to exception #1. If a disconnect from a bus duct serves a motor mounted at 10 feet high, for example, the disconnect can also be 10 feet high if it is located by the motor. The idea is that if someone can reach the motor to service it, they can reach the disconnect also.

The situation intells providing 480v to a transformer that feeds a series of panels for branch circuiting. The transformer is fed from the bus plug. I think 368.12 will provide the answer I need. I just want to insure that the bus plug can be switched to the off position with out having to locate a ladder to do so, in an emergency.
