disconnects switches for miscellaneous equipment

In a hospital lab building, I've designed the following feeder to a 10kVA, 230V, 1PH irradiator:

From a 208Y/120V panelboard, I've got a 90A-2P breaker feeding 15kVA 208V to 230V, 1 phase transformer located immediately adjacent to the source panel. So, a 125% primary OCPD. From here I go to the irradiator at which I've place a disconnect. I made it fused but that's not really necessary.

The client has precious little space in the room with the irradiator and would like to forgo the installation of this disconnect. Choosing instead to just connect directly to the equipment.

This is a bit counterintuitive for me as I would normally put a disconnect there for the sake of working on the equipment were it necessary. But I don't think it's it's a code violation. My questions are these:

a) Do you agree that it would be code compliant without this disconnect?
b) Is the above question contingent on the primary device being lockable and if so, would it have to be lockable with adding special locking device?
c) The irradiator does indeed have it's own main breaker. Does that matter either way?

a) I am not real familiar with hospital wiring or equipment but unless the code requires it, I can't see any reason at all to have an extra disconnect.
b) you are not required to have a disconnecting means lockable or otherwise, unless the code says you have to. Personally, I would be adding the 10 cent breaker lockout kit to the feeder CB.
c) How would you work on the main CB in the irradiator without being able to lockout the feeder anyway?
