In this illustration i am assuming the voltage to ground is 277V, that is why we need to follow the requirements of table 110.26(a)(1) to place the equipment so the edge of it is at least 3.5' away from the door.

Yes, I agree that the assumption is that voltage to ground is greater than 150 volts due to the 3.5' dimension.
