Do I need a breaker before the wire enters the building?

I am upgrading my service to 400 amp to accommodate an ADU (accessory dwelling unit) that will have an electric water heater. I purchased a 400 amp meter main combo with feed through lugs directly on the 400 amp buss bars, intending to run either a 100 or 200 amp circuit to the panel in the ADU. There will be no breaker/disconnect at the meter/main for the ADU, only the 400 amp disco. The conduit for the ADU electrical panel is roughed in through the partial retaining wall, and will go to a panel inside the building.

Is it acceptable to run wires of less ampacity directly off of the 400 amp buss?
Does the main disconnect for the ADU need to be outside the building, or can it be inside?
Does the disconnect for the ADU need to be it's own breaker within sight (less than 50ft), or will the 400 amp main be acceptable?

The reason I'm trying to do it this way is to avoid buying a 200 amp breaker for the meter/main, and then likely have another 200 amp disco in the sub panel.
