Double Duplex Receptacle

If I have a double duplex receptacle that I feed each duplex with a separate circuit and a separate neutral, do I need to have the circuits have a breaker tie?
 
Not if each yoke or strap is served by only one ungrounded conductor.

Clarification: should have said not if each yoke or strap is served by only one two wire circuit. If two yokes have a separate ungrounded conductors and a common neutral then you have a multiwire branch circuit (or some circuit that doesn't meet MWBC requirements) and still would need a handle tie.
 
I was told once that if you install two devices on separate circuits (different phases) in the same box, that your device has to be rated for the available voltage in that box.

ex: You have a duplex fed from a 120/208V single phase panel. One recep is fed from phase A, the other fed from phase B w/ a separate neutral. Are the receps required to be rated from 208V, or is 120V enough?

I tried to find a reference but I couldn't, is there any validity to this? If so code ref please....

Not exactly what you were told but, see 404.8(B)

(B) Voltage Between Adjacent Devices. A snap switch shall not be grouped or ganged in enclosures with other snap switches, receptacles, or similar devices, unless they are arranged so that the voltage between adjacent devices does not exceed 300 volts, or unless they are installed in enclosures equipped with identified, securely installed barriers between adjacent devices.
