doubling over wires to fit in a mechanical lug

I know that mechanical lugs must be used only for listed purpose but does any particular code section other than 110.14 address the issue of wire termination? More specifically, this picture is from a small sub-panel in our facility. The lug is too big for the wire so the contractor umpteen years ago folded the wire to terminate instead of changing the lug to a smaller one. This panel used to have an isolating xfmr installed with larger wire that is no longer in service.

There is nothing in the Code which directly addresses that. Indirectly, 110.3 "(B) Installation and Use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling."

If the lug is properly sized then folding is not necessary. IMO this deserves a red tag every time. :)

It is likely perfectly safe though. Certainly safer than using the same lug and wire combination and not doubling it up.

I would not be getting real excited about it. You could "fix" it if it really bothers you.
 
With all that aside,,,,,, That is a perfectly clear Picture,,,,, what did you take that with?
