Have a question, what cause the 20 years old dry type xfmr extreme hot and making a lot of noise? wiring insulation issue? Its t a signal of its lifespan come to an end?

Newer transformers are subject to more stringent energy efficiency requirements. So even when it was brand new 20 years ago, it was probably less efficient than one you would buy now. But replacing it will never pay for itself in energy saved.

Could you possibly assume a worn transformer like this would deliver maybe 5% lower voltage to the secondary?
It's hard to imagine how the turns ratio could change over time. Typical losses are in the 2-3% range. Losses great enough to cause a 5% voltage sag would probably include letting some smoke out.
